HEART DISEASE & STROKE PREVENTION

Program Objectives

1. Partner with allied health professionals and medical professionals to implement intervention strategies that promote primary and secondary prevention of heart disease in the areas of complementary and alternative medicine.
2. Use educational and motivational strategies to provide program adherence and successful reduction in measurable cardiovascular disease risk factors (i.e blood pressure monitoring during activity, cholesterol, cardiovision).

Program Details and Interventions

Nutrition
· Heart healthy and low sodium smart templates and online food and nutrient tracking

Exercise
· Prescribed aerobic and resistance training program
· once per week (minimum) monitored resistance and aerobic
· Prescribed home exercising
· Blood pressure monitoring during activity

Medical
· ECG as needed
· Physical exam prior to introduction of program
· CVD education package

Program Goal
The primary goal is reduce cardiovascular disease and improve quality of life. Each year approximately 57,000 Canadians die of heart disease and is the second leading cause of death, next to Cancer.

This program uses primary prevention interventions to reduce the incidence of heart disease and stroke and uses secondary interventions to reduce the consequences of heart disease and stroke.

Target Population
Every population can benefit from this program; however the primary target population is men and women at risk of heart disease and stroke.
